Turned out it was the SATA controller (the motherboard's) that failed. Any CF cards are mounted as IDE devices, even though they are really connected via USB. They checked everything we did, drivers, diagnostics, etc. Resolution: Remove or replace the faulty hard drive. Check This Out

At last I resolved it successfully. Sure enough, removing that drive generated the error.
